Registration with the practice is restricted to patients who are moving into our practice boundary as noted on the map below. To register, you need to complete a registration form obtained from our reception desk, or by clicking on the link to the document below. You will also need to complete the relevant health questionnaire, also linked at the bottom of this page. Please be thorough and complete all sections of the registration form, as unfortunately we are unable to process incomplete forms, and this may delay your registration. We aim to process registration applications within one to two working days but this may take longer.

During this pandemic please email forms to clinical.s70639@nhslothian.scot.nhs.uk  or post in prescription box inside practice door. It is the applicant’s responsibility to prove their entitlement to NHS Services when applying to register. Where there is any doubt, for example, if you were born or have lived abroad for a considerable period, the staff may ask for further documentation such as a passport, medical card or evidence of permanent or regular return to the UK, if this has not already been presented. This process follows NHS Lothian and NHS Scotland guidelines and we would be grateful for your cooperation in this routine administrative matter.
